Titans News · Berea-Midpark Athletic Training Staff to Utilize Healthy Roster


The Athletic Training Staff at Berea-Midpark High School and Southwest General Sports Medicine currently use Healthy Roster as our injury management system.  It will allow injuries to be done paperless, allows you the parent up to date status of your child, upload notes, notifies the coaching staff of activity modifications and many other features.   You will be able to notify the staff of injury, update documents such as physician notes, and communicate with the athletic training staff. Physicians on the system will communicate directly with us, and upload any required referrals on the system.  You as a parent will not have to input any information, the Healthy Roster will sync with Final Forms,  which  will still be used for other administrative documentation such as dates, signatures and so on.  This is an injury management system only, students not on any BCSD athletic team will not be using this platform.  If you have already signed up with another sport or previous year, and contact information has not changed, you can disregard this notice.

In the next day or so, you will be invited to either sign up then download the app to your phone.    Please find additional information from Healthy Roster attached.  It is not required that you join although recommended to say current with your child’s progress with injury management.
